Occupational / Physical Therapist
This full-time position will be responsible for assessing, planning, implementing and evaluating therapy programs in order to restore function and prevent disability in home-health patients. Will work in collaboration with patient’s physician and interdisciplinary team members.
Successful candidate will have a minimum of two years experience, preferably in-home. Current license to practice in Florida, Florida driver’s license, reliable transportation, current CPR certification, and state-mandated education requirements. As well as, Electronic Medical Record experience.

Registered Nurse
This full-time position is responsible for assessing patients utilizing history and physical assessments; develop plans of care; as well as provide nursing interventions including direct care and patient education. RN is responsible for collaborating and communicating with team members, as well as documenting patient data.
Job Requirements: strong communication skills, knowledge of computers and Electronic Medical Records, and ability to work independently. Current license to practice in Florida, valid Florida’s driver’s license, and reliable transportation. Minimum of one (1) year experience in medical/surgical, home health or hospice nursing preferred.
